Who We Are

Morgan has been the CEO of Your Medical Home since 2021. Under her leadership it has grown from a single clinic to five locations and nearly 50 employees. An entrepreneur at her core, she thrives on solving complex problems, creating new opportunities, developing talent, and scaling organizations. Her career spans healthcare, technology, and senior living, building platforms and partnerships that turn ideas into operating businesses. Morgan holds a B.S. from Texas A&M University and lives in Austin with her family.

Adam Jaynes MSN, FNP-C graduated from Georgetown University in Washington D.C. and Lamar University. He has been a certified Family Nurse Practitioner since 2016. Since that time his primary role has been serving the Southeast Texas community in occupational medicine. Adam is a lifelong resident of southeast Texas and is passionate about caring for this community's members. He was born in Beaumont and raised in Orange where he graduated from Little Cypress-Mauriceville High School. When not at work he enjoys spending time outdoors with his wife Carly and their three children, especially deer hunting and fishing.

Leslie Gray is Your Medical Home's Director of Human Resources, Executive Administration, and the Director of the Community Care Clinic. She serves on the leadership team and connects with every team member through her role in HR. Leslie has over 20 years of management and HR experience. She specializes in organization, strategic planning, and business and staff development. Leslie works from her home in The Woodlands and enjoys time with her family and traveling.

Emily Klucik is Your Medical Home’s Director of Marketing. She has been a licensed X-ray Tech since 2013 and began her career at Your Medical Home 8 years ago. She serves on the leadership team and has worked in and managed several different departments throughout her career at YMH. Emily has lived in Southeast Texas since she was 2 years old and is very involved in several different organizations in and around Bridge City. Outside of work, she's most likely at a baseball field or basketball court watching her kids in their element, or at her favorite vacation spot, Disney World.

Jennifer Theriot has been with Your Medical Home for 10 years and serves as Director of Health Care Administration, where she oversees billing, compliance and clinical operations. She is the operational backbone of Your Medical Home — the steady force whose systems, judgment, and problem-solving make the organization's day-to-day success possible. Known for bringing order to chaos, she translates complex regulations into the workflows that empower staff to deliver compliant, compassionate care. Outside the clinic, Jennifer is a devoted mother and grandmother whose family is the center of her world.

Summer Rose Recendez is the Director of Accounting at Your Medical Home, bringing more than 13 years of experience in financial management and operations. She oversees budgeting, financial forecasting, and accounts payable and receivable, safeguarding the accuracy and efficiency that underpin the organization's financial stability. Outside of work, Summer enjoys hiking, kayaking, reading, and time spent with her husband and family. She is also active in her community, volunteering with local animal shelters.
